It was transcribed by John Rolad Penner in 1994 from a small typed booklet, then photocopied and stapled. The booklet includes no means of contacting the publisher, although the name 'Kolmogorov-Smirnov Publishing' appears after the title page. The only form of date identification is the hand-written purchase date: April 29, 1978.
